Information for "self-pay" patients

All patients who have preregistered their email addresses with our Contact Lens Department, and are self pay, can order their lenses online.

Patients who have not preregistered their email address with our Contact Lens Department, please email us with the following information to be registered for online ordering services:

• 2. address
• 3. Phone number
• 4. birthdate
• 5. email address.

Information for patients with insurance

If you already have insurance and want to order contacts, please email us. When ordering contacts via email please include the following information.

1. Patient name
2. Address
3. Phone Number
4. Birthdate
5. Number of boxes to order
6. Insurance Provider
7. Last 4 digits of Social Security Number
